Information for physician offices making referrals
To make a referral:
For your HPN patients, please submit a referral authorization to HPN. When it is approved, please send:
- The latest progress note that covers the patient’s pulmonary condition;
- The most recent chest x-ray (film and report);
- The most recent breathing test/PFT report.
For your Humana patients, please submit a referral authorization to Humana. When it is approved, please send:
- The latest progress note that covers the patient’s pulmonary condition;
- The most recent chest x-ray (film and report);
- A breathing test/PFT can be performed at Pulmonary Associates, if the patient has not had one recently.
For patients referred for allergy evaluation, please send any recent clinical laboratory allergy testing panel results.
For all other patients, please send the most recent progress note that covers the patient’s pulmonary condition.
